A 1-year membership agreement is required, followed by a month-to-month agreement.

Automatic payment: Payment will be automatically deducted on the 1st of each month. (Eg. If you sign up on April 8th – Your membership starts on May 1st)

Membership benefits and credits are nontransferable or refundable and cannot be shared with or redeemed by others.

Member has the option to opt out of monthly treatments for maximum of two months, and use the $358 (plus applicable taxes) towards a toxin treatment (Botox, Nuceiva, or Xeomin.). This option does not apply to any other service or product.
