Almost everyone who comes to us about pigmentation has already tried to fix it themselves. Brightening serums, vitamin C, a peel kit ordered online, something a friend swore by. Sometimes it helped a little. Often it did nothing. Occasionally it made things noticeably worse.
There is usually one reason for that, and it is not the products.
Pigmentation is not one condition
This is the thing that changes how people approach it. Dark spots, brown patches and post-breakout marks can look almost identical in a mirror while being fundamentally different underneath. They sit at different depths, they are driven by different triggers, and they respond to completely different treatment.
A product formulated for one will underperform on another. A treatment that clears one type quickly can aggravate another. Buying a brightening product without knowing which type you have is a coin flip.
Learning to tell them apart
You cannot diagnose your own skin from an article, and you should not try. But knowing roughly what you are looking at makes you a much better judge of the advice you are given.
Sun-related spots tend to be discrete. Defined edges, distinct from the skin around them, and clustered where you have caught the most daylight over the years. The face, the backs of the hands, the chest and the shoulders. They accumulate slowly, which is why they seem to appear all at once in your forties. They have generally been forming for two decades.
Hormone-related pigmentation looks different. Larger patches with softer edges that blur into surrounding skin, often roughly symmetrical across both cheeks, or the forehead, or the upper lip. It tends to arrive alongside a hormonal change rather than accumulating gradually, and it is the type most likely to worsen with heat and light. This is also the type where aggressive treatment most often backfires.
Post-inflammatory marks are the ones left behind after something happened. A breakout, a scratch, an irritated patch, a burn. They sit where the inflammation was, and the giveaway is that the skin texture is completely normal. These are pigment, not scarring, and many of them fade on their own given enough time and enough sun protection.
Why the wrong treatment makes it worse
Light and laser treatments work by being absorbed by pigment. That is the entire mechanism. Energy goes in, the pigment absorbs it, the pigment breaks down and clears.
Which means the thing that makes these treatments effective is also what makes them risky. If the settings are wrong for the skin, or the type of pigmentation is one that reacts badly to heat and light, the same energy that should clear pigment can provoke more of it. The skin responds to the treatment as an insult and produces additional melanin in response.
This is the single most important reason pigmentation should not be treated from a photograph or a standard protocol. Matching the approach to the skin type and the pigmentation type is not a formality. It is the difference between a good result and a worse starting point.
It is also why any clinic that offers you a pigmentation package without looking closely at your skin first is doing it wrong.
The part that does most of the work
Here is the thing nobody wants to hear: sun protection contributes more to your long-term result than the treatment does.
Treatment clears pigment that already exists. It does not remove the cells that made it. Those cells are still there afterwards and they respond to the same triggers they always did. Without daily protection, you are clearing pigment while continuing to produce it, and the maths on that does not work.
Daily broad spectrum sunscreen, all year, reapplied when you are outdoors for any real length of time. Indoors too, if you sit near windows or in front of light-based devices. It is unglamorous and it is genuinely the highest-leverage thing available to you.
The same applies in the week after any treatment. Keep things gentle, hold off on actives unless you have been told otherwise, and do not pick at spots that are darkening and flaking. That darkening is expected and that flaking is the pigment leaving.
What treatment can realistically do
Several approaches are used, and in practice most plans combine more than one.
IPL uses broad-spectrum light to target pigmented cells and improve overall clarity. Laser treatment works more precisely, breaking down excess melanin so pigment fades without damaging the skin around it. Which of these fits, and at what settings, depends entirely on the assessment.
Plasma treatments contribute differently. Plexr® Plus Plasma stimulates skin repair and improves texture, while CoolJet Plasma infuses active ingredients deep into the skin to hydrate, calm and strengthen the barrier. Barrier strength matters more than it sounds here, because irritated skin pigments more readily than calm skin does.
Exosome therapy and red light therapy are both used alongside these rather than instead of them, supporting recovery and calming the skin between sessions.
One clarification worth making: not all discolouration is brown. If what bothers you is persistent flushing or facial redness, that is a different concern with a different plan, and there is a separate rosacea and redness approach for it.
Setting expectations honestly
Pigmentation responds gradually. A course outperforms a single session, and going harder and faster generally works against you rather than for you.
Some types clear well. Others are managed over time rather than resolved once, and being told that at the start is more useful than discovering it three sessions in. Results vary from person to person, and the honest answer for your skin comes from someone looking at it. Consultations at MD Laser Clinic are complimentary.
